  • WHO CAN GET THE VACCINE?

We vaccinate all of our patients and, for flu season, we also vaccinate all caregivers of our patients. This includes: parents, guardians, grand-parents, step-parents, or nannies. (please read below to know what adults must bring):

    • We will not vaccinate families who are NOT patients at Midland Pediatrics.

    • We encourage those patients who qualify for (TVFC) state vaccines (self-pay patients & underinsured patients) to still come inside the clinic during shot clinic hours to receive your vaccinations. The vaccines being given are exactly the same; however, the vaccines from the state may only be given to those patients who qualify.

    • All adults who get the vaccine from Midland Pediatrics will be required to complete a consent form. This form asks specific medical questions to which the answers may cause us to decline administering the vaccine here and encourage you to consult with your own Physician for flu vaccination.
  • WHAT DO I NEED TO BRING?
    • We will need parent's drivers license & insurance card for every adult who wants a vaccination. We will only need the current, active insurance card for children.

    • Please wear appropriate attire that will give the nurse access to administer the vaccine in the upper arm. For very small, younger age children it will be administered in the upper thigh.
  • HOW DO I SIGN UP OR SCHEDULE?

Call the office at 432-620-8687. Our receptionists will be happy to assist you and schedule the family for a time slot during a curbside flu clinic.

Signing up or providing paperwork ahead of time is not required; however, you may print off the parent consent form and bring it with you completed if you like. Otherwise, a blank one will be provided to you to be fully completed before the vaccine is administered.
